The following Eye Examination Report form is available for informational purposes only. The Florida Highway Patrol requires that all candidates for the position of State Trooper have vision which is correctable to 20/30 in each eye. If you believe that your vision may not meet these standards or you would prefer to discuss the report with your ophthalmologist, please download and print the form.

Each candidate who has successfully passed the polygraph examination will be sent to a certified state of Florida ophthalmologist. This test is offered at no cost to the candidate.
